  2. Indicate if you feel your peripheral vision is affected and in which eye.
  3. Is there any pain or discomfort?
  4. Is there any redness?
  5. Is there any discharge?
  6. Are the eyes sensitive to light?
  7. Do you have a headache?
  8. Have you noticed any floating specks or flashes of light in your vision?
  9. How long have you been aware of the problem?
